STUDENT SAFETY PLEDGE FORM

__________________, who is enrolled in Vocational _____________ will as a part of the shop experience, operate implements and equipment providing that her/his parents or guardian has given written permission.

It is understood that each student will be given proper instruction, both in the use of the equipment and in correct safety procedures concerning the equipment, before being allowed to operate equipment alone. The student must assume responsibility for the following safety pledge.They will be tested on safety with 100% score.

1. I PROMISE TO FOLLOW ALL SAFETY RULE FOR THE SHOP

2. I WILL NOT ASK PERMISSION TO USE A PARTICULAR IMPLEMENT UNLESS I HAVE BEEN INSTRUCTED IN ITS USE.

3. I WILL REPORT ANY ACCIDENT OR INJURY TO THE TEACHER IMMEDIATELY
